The transfer of authority from engineer to administrator; passing the buck; PowerPoint engineering and poor management from senior officers to name a few.
The saddest thing for me is his acknowledgement of the similarities between the XV230 tragedy and the Herald of Free Enterprise, NASA Space Shuttle ‘Columbia’, the Zebrugge Disaster (1987), King’s Cross Fire (1987), The Marchioness (1989), and BP Texas City (2005). When are we going to start to take safety seriously?</description>
